If you go to the next image on Ancestry.com it turns out to be Shelby's older brother Russell Dillman born Sept 27, 1897 who registered on the same day. The handwritting on the card is drastically different from that on Shelby's which suggests to me that they filled out the cards themselves but that is just a guess.
